Front End Issues in a Springer?

Default Front End Issues in a Springer?

I was looking at my bike today and noticed that there is something that looks messed up in my front end. It's hard to explain, but if you look at the pictures, the stem that goes through the neck looks like it is messed up. Is this supposed to be this way? It's like the steering stem isn't square to the forks. Any help would be much appreciated.

I found the Acorn nut on the steering clamp is loose. Not good.

My buddy has a '08 CB that has the shakes at low speed..

Any ideas???
