Default Transmission problem

I have a 2007 Heritage with approx. 53k miles. At around 30-35 k I noticed a couple times that when coming to a fast stop and breking hard that the transmission sounded like it was going to disengage the gear. This is now happening more frequently. Also when the engine is really heated up after a long ride that there is a very faint clunking noise in the tramsmission when stopped. Is this an indication of the clutch pack going bad or something more serious. Any help would be appreciated.

Replaced the hose's and filter inside the tank at 33K...
