Default Loosing Clutch

Installed a new clutch in the 1980 FLH. Set everything to the book. Took it out for a test ride, got about 5 blocks from home and lost the clutch. I had tons of cable slack at the handle bars and a clicking noise that seem to come from the lower right side of the bike. Got it home reset everything again and the same thing happened, but no more clicking noise.

Any suggestions?

Here are a couple pics of the push rod ends. I don't know what the ends should look like.

The right side rod end looks pretty rough, I would guess your throw out bearing took a crap.

That's kind of what I was thinking. Never had the trans cover off or replaced a throw out bearing before. I looked through the service manual, there is not a lot about throw out bearings in there. How difficult of a job is this?
