I had the same problem. Back in my adolescent days I worked at a bicycle shop. I just transfered my knowledge to motorcycles. Place two small strips
of 100 grip sandpaper around the bars under the risers, and problem will be resolved. Since I've put the sandpaper under the risers, the bars haven't budged.

Did you keep the stock rubber bushings installed or did you replace them with the polyurethane bushings? I had this problem until I did two things. Torqued down the top bolts really good with a ratchet and not just a allen wrench and installed the polyurethane bushings. The bushings take all play out of the handlebars which helps them not come loose.
