Just switched my wheels out for a set of Southern MCs and i decided to go wider in the front with a 3.5x21 running a 120/70 tire.

When I mounted the wheel it looked off center, but i thought it was because of the fender being smaller, I was working graveyards and tired, and the rotor lined up in the caliper so I didnt do any measuring then i just blew it off. Well I went out there tonite to do some measuring for a wrap fender and decided to measure fender mounting points to lip of rim, and sure enough its off. It sits about a 1/4" to the brake side. Like I said the disc lines up in the caliper and the caliper is mounted to the fork leg, but is this normal when switching to a wider rim?

Can this cause any issues? Bad tire wear? Anything..

Because of the wheel being shifted over the the right more the fender sits lower on the tire, enough to knock all the new tire hairs off on that side.

the tire does not have to be dead center in the fork tubes it is the same as the rear tire it is all based off motor balance and alignment. If you have the rotor in the caliper center, did the proper torque sequence when installing, your spacers are correctly installed, and the new wheels hub uses the same stock spacers you are fine

ok what about when I do a stretched fender eventually? just use different size fender spacers to make it even?

yes sir. just cut spaces to match to get the fender where you want it

Same here my front is not center it's a bit off to the right
