Default Problem Shifting and disengaging

when I pull my clutch the foot pedal will not switch gears unless I give it throttle. also the slack on the clutch changes as I ride.. sometimes it goes all the way back and sometimes theres too much slack. some times when I pull the clutch it doesn't completely disengage and if will stall if I come to a stop. I have adjusted my clutch and replaced the cable and the problem still exists. PLEASE HELP!!!

+1 and make sure the adjustment is done with the powertrain cold.

Im going to order a plate kit, a spring, and a throw out bearing... just need to figure out which one of the two throw out bearings I need.
