Default clutch adjustment

I need some help on my 89 sportster 1200. While riding home last week my clutch cable lost its adjustment. I did the adjustment by the manual, adjusted my cable to put all slack back in, removed the clutch inspection cover, removed the spring and retainer, turned the adjustment screw in to tension then backed off 1/4 turn replaced retainer, spring, and inspection cover, and adjusted the cable for 1/16 - 1/8 of an inch slack. I took the bike around and within a few miles the cable went slack again. I don't think its m clutch as the bike isn't shooting up RPM's. I replaced the throwout bearing and re-did the adjustment and took it for a ride and again within a few miles the cable went slack again. Is there anything I'm missing or anything else I should check?
