Joker Machine Large Round Bar End Mirror
Comes with the bar end adapter to fit perfectly onto the end of the RSD clip ons.
Great mirror, it has a convex surface so you get some better peripheral visibility from it. Superior to a lot of other bar end mirrors (IMO)

I don't know exactly, but I can safely say that it is less than 100 miles. Honestly, I hardly ride my bike lol.
