It's in the Appendix of the HD Service Manual for your model, and it's a double sheet folded overa pita to scan.

My dealership allows me to "borrow" the Service Manuals, Electrical Diagnostic Manual, Parts Catalog, etc. that I need (I already have the SM and Parts books for my bike, but I frequently do projects requiring other information)and copy the pages I need in the sales dept.
