Try replacing the oil cap first. Mine did the same thing till I broke down and replaced it with a new cap. As they age it seems they don't hold as well.

Kinda looks like an FXR/Dyna model. What type of revtech is it?

2003 revtech 100 Evo is all I know

Is there anything I can look for on the frame or the motor to better identify it?

Did some reading BMC(Big Mike's Choppers) was a short lived company in Oregon,now dead. They made only a few models before bellying up. All of them used revtech.

you were able to find a little more than I did but the location lines up. Bend Oregon. I sent a message on the website a week ago but never heard back. Going out of business would be a pretty good reason why

Here the article that describes the closure. Apparently Big Mike's choice of Revtech soon led people to other builders like Big Bear Choppers as the article points out. That led to him going out of business.

Well if you want to keep close to same size it will cost ya. S&S has 4 Evo engine sizes 80,96,111 & 124. Depends also on the finish too.
