Make up a "mock-up" of what you want done, full size, if possible. A really good, high pixel jpeg will do fine, too.
Go to your local vinyl sign printers and ask them to make it for you. If they go on about "Copyright" crap, remind them that if you're not selling the thing, then they can, indeed, print it for you. The supreme court said so a number of years ago.
If shop #1 won't do it, I'm sure you can find one that will.

Had a guy at work that would make anything U wanted for personal use..
made a lot of decals for the guys that fished...for their boats and trucks
