Haven't had it happen, but I can see where it could.
Not sure about the Tri-Glides, some of the inserts are held on with 3M tape (the insert with the bolt hole), and some are held on with Velcro(no bolt hole).
Especially the Velcro style, if the insert isn't pushed in hard enough so the Velcro connects, could easily fall off.
Good news, is that they're inexpensive, around 18 bucks I think.
