I asked two MSF instructors on 2 different sessions (basic and experienced). \\; They both said the left (hammer) lane is the safest, not the HOV in some big cities, though. \\;
Since in the left lane, you just have to look out for merging traffic from one direction, the middle lane is worst, merging traffic from both left and right. \\; And in left lane, traffic might cut into your path rather abruptly, esp they are usually accelerating coming in and slowing down going onto exits.
I usually stay in the left lane and "haul ***" @ speed limit or 5-ish over.

5 Over is not fast enough for the right lane here in Jersey. If you want to ride in the left lane 75 is the slowest you will be going no matter if the limit is 45 or 65.
I try to stay in the right lane and then look out for merges from the right. If there is room I will move over to the middle till I get past the merge then get back to the right lane.
