Found it in a yard shed where it'd been sitting for 5 years (with half a tank of fuel). Tore out the mouse nests, removed the sheet metal, and rebuilt it.

S&S Super E, Accel Single-Fire electronic ignition.
Suspension Modifications:
Chrome wide-glide front end.
Wheels/Tires Modifications:
21" front wheel, chrome spoked wheels.
Interior/Exterior Modifications:
Custom dash with built-in mini-speedo & tach. LED turn signal/high beam indicators.
