Had the shop install, mount and balance my rear tire.
Rode it for 2 weeks straight then went on for a 6 hour ride to which my bike started weaving side to side 4 hours in.
looked at my rear wheel while rolling, both rear axle nuts are spinning. They spin forward when going forward and backwards when rolling back.
It looked like the axle was spinning outward to the left. But some say it could be the bearings locked up.
Pull it apart and find our.
Zooming in, the seal definitely appears to indicate a bearing issue on the caliper side. Drive side really is not visible.
How many miles on bearings?

I agree with @Andy from Sandy , since it looks to me like the nuts aren't tight enough, so they're rubbing against the swingarm, causing that arced "wear" around them. I'd definitely check the torque on them and make sure it's right.

Bike is at my buddys house 6 hours out. Waiting on him to pull it apart.
Bearings never changed since I bought it around 20k miles, it now has over 33k.
I'd be having a conversation with the shop that did the work. Even if they accept no responsibility, it seems like someone dropped the ball on that one and they should be aware of it.
There is some thing to say to the shop that had it apart.. my Dyna when a bought it had 21k on it and I felt a knock on the front.. bought a puller and replaced them myself.. Also did the rears.. cheap insurance.. $26 a set.. On my Evo and Shovel, every time I replaced a tire the bearings were cleaned and repacked..
