You disassemble the steering head and repack the bearings.
The service manual will show you how.
The grease fittings on steering head of the frame were iffy at best in greasing the bearings.
I only had to remove a few parts to get at the steering head bearings on my old Softail.

My bike is an '05 Road King, and on mine there is a Zerk fitting on the left side of the neck (as your sitting on it) close to the front of the neck. You have to get down on one knee to look under the windshield to see it. It's the only one on the bike. You won't be able to get a straight grease gun fitting on it, you'll need a right angle version to grease it. Even then it's a PITA. Harley used to have 3 Zerk fittings on the older touring bikes. 1 on the inner primary cover for the shifter, and one on the lower side of the brake pedal, as well as the one on the neck.,,,
