Just an update for anyone who is interested. MoCo honored Medusa's warranty, all the parts are in. They will begin to rebuild on Monday, an entire top end as well as oil pump and crank assembly. Plus assorted seals gaskets bearings yadda yadda yadda.
Also installing RSD Venturi 7 spoke AC, black powder coated push rod tubes and Road King shocks.

So what do you think was the main cause of your problems?

Several issues, the main one being that the pistons were not set right, causing them to score the cylinders.
