I’d trust Dan's counsel on this.

Looks like the washer is too thin. The cotter pin barely engages the castle nut grooves. Might throw a lock washer in that stack, too.

Better yet, find out what part is not correct...

The washer the op shows looks correct, but I agree that something doesn`t look right about the cotter pin not engaging the castellated nut, it may be as simple as the wrong nut being used.

@PLANET HARLEY You should measure the length of the spacers (dimensions are in post #6).

How long have you had this bike?

Did you remove the grease seals and clean and inspect the bearings?

You need to correct offset retain and probably the correct washer. You present application is not safe.

You could if you insist use a larger cotter key pin to fit hole and nut slot but rotate it 90 degrees. That's the proper way for that type of nut.

Then wrap one end over face of axle and other over hex flat cutting it correctly to length. The pin loop has to be in slot and the exit pin in a slot. If not, get a thicker washer for sure.

Here is a pic of the nut on my `89 FLSTC.

The axle protrusion through the nut is close to OEM, but I am running an aftermarket caliper bracket so it is not exactly the same stack up.

The washer, axle, and nut are OEM.

Yes, I will measure the lengths of the spacers.
I have had the bike for 1 month
Do you have the part number for the washer and nut?
Thanks

Nut part number: 7987

Washer part number: 5756

These parts are still available from the dealer, but probably would have to be ordered.

You can also find them from any of the major aftermarket outfits.
