Need help identifing engine problem

I have a 2004 1200 custom and she keeps cutting out in low-mid RPMs so bad that the check engine light comes on, it pops and cracks intill it hits about 3000 RPM then it takes off like a bat out of hell. In really low RPMs it runs fine but as soon as i hit about 2500 it starts cuttin out. It only does it after its warmed up, when its cold I have no problems.

Please help me out here. Thanks

If the check engine light is coming on, then the computer is very likely storing DTC's, and that should point you in the right direction. Have you tried reading them?
