I went with 16" Carlini's, which are actually closer to 17's and I went +8" on all cables and brake lines and +12" on the electrical extensions. You can probably use that and knock off a couple of inches for shorter bars. Good luck!!
I have installed WO 14" Apes on an 06 and an 08 RK and used +8 from Magnium Shielding for both the Clutch and Brake lines.
Be careful about using +6 on bikes with ABS as the mid lines are all the same length. The difference in cable length is taken up from the perches to the midline connection. I tried a +6 brake line on my 08 RK and it pulled the midline connection just enough that it showed where it came out of the Nacelle. Fortunately I only had to purchase one new section.
Again this was with Magnium Sheilding cables on a RK so I'm not sure this would apply to your Street Glide but it's something to think about. You want to have enough cable that the connection is hidden
