My bike is exactly the same, plus my rear wheel is offset to the right. You'll find a lot of good threads about alignment here. I'm gonna tear into mine pretty soon. The front end has to be aligned first, then the drive train.
My old dirt bike used to do it all the time. I would put the tire between my knees and twist the bars back straight. Probably not the best thing to do but it worked.
My guess is the forks in triple tree are tweaked a little. Maybe one of the forks twisted a little in the clamp. I'm by no means an expert but I'd either take it in to a pro or I'd loosen the fork clamps slightly, twist it straight, then tighten them back up.

Yeah, ummm..... no. There is nothing to index the lower fork slider to the fork tube. You can spin the lower fork slider 360° on the fork tube.
Yes that would be a good choice. Even more important is making sure you have a one-piece top clamp. I don't know what Road Kings had but my FLHT had a 2 piece which can cause a busted bar on apes. I went to the Yaffe one piece clamp with 3 set screws to lock the bar in place. it will not move with this clamp.
