Twice, not once but twice!
Ram mount cup holder. Driving from Kremmling CO. to Gunnison CO. last summer. Going over McClure pass and my cup holder decides to leave the bike.
Screw that attaches swivel holder to mount had backed out while riding!
Took 3 calls and numerous pictures and 6 weeks to get them to warranty.
Fast forward: On our way from Cortez, CO over to Utah, Wife has a Ram cup holder on 2014 SGS. Cup holder falls off, bounces on tank and lands in her lap. Chipped her clear coat and wrecks her day.
The screw shows remnants of blue locktight but my advice is back screw out and install red locktight yourself!!

That stinks! Never had an issue with the Kuryakin model...I was never a fan of the ram mount cup holder, but that's why they make all kinds of them, different choices
I just purchased two. Both had the screw loose out the package. So I pulled the screw and it had blue lock tite on them. The lock tite was not even disturbed. So this made me think the screw and hole are different sizes. I just filled the hole with loctite and wiped off the excess. Watching to make sure it stays put. If not I'm putting a new screw.

Sorry to hear yu're having problems with yours. Mine is 3 years and almost 14000 miles. Still good as new. The only thing I don't like about the RM cupholder is that it seems to take up a lot of room.
