I think it's from V-Twin Manufacturing. The rubber ***** are from Amazon and I over reefed them a bit which mangled them a little; small potatoes, I ain't got time to grieve for that.
Mounted it up this morning and just tested it, it takes some getting used to. I would prefer the back end to be about a 1/2" longer, I think the leverage would suit me better.
I like the ole' heel/toe on past dressers and figured what the hell, give it a try. I may turn it clockwise a notch, wouldn't be surprised if I don't keep it, it's all about sheets and greens anyway.

I had to stop using the 5" mini footboards that I really liked. Not enough to take it off though.

Been using this one for almost 4 years now. Got in a wreck and broke my leg, had to get a rod an 4 screws. Leg hasn't been the same since, this helps alot.

On my 2000 FXD I used a transmission shift lever and welded it to the oem shift lever that came on the bike. I plugged unnecessary holes with some grommets from Lowe's. Originally, I had a heel hammer but found that there wasn't enough leverage for heel shifts and this setup leaves enough room for my boot heel to fit between the peg and the lever.
