My 04 Dyna low rider is shifting rough, every gear change I hear a big clunk. I know they are loud when shifting but it seems like it has gotten worse.
only 14,000 miles on it, changed all 3 holes with ams oil, primary and tranny fluid.
Any ideas what it could be?

14,000 miles is not much for a Dyna tranny. Are you the original Owner? The transmission does require maintenance but, it's nothing but a gearbox, if you change the oil/fluid as required it will work well for a long time. The first (break in) oil change is probably the most important, it needs to be done on time.
The First and Second trouble shooting steps have already been mentioned above.
I suggest you go on Amazon, Ebay, etc. and find an H-D service manual for your year model bike and follow the required maintenance procedures. It has trouble shooting guides in it as well, Good Luck.
