I have same problem which I put down to the design of the lever.
Because the the lever metal is "hard" the slot does not contract enough when you tighten the Allen screw. I know that silver foil or similar has been suggested in the past. I believe that a nut & bolt through the lever,instead of the Allen screw would tighten the lever much better although it wouldnt be as neat. Alternatively drilling through lever boss/flange & shaft, then a "pin" fitted could help. A heavy duty attachment to give you more leverage when tightening the Allen screw may also help. I notice in your pic. that the lever boss/flange is protruding slightly from the shaft end.
Check out some of the local bearing wharehouse for either a brass bushing or perhaps if tyou have enough material and can find a small enough needle bearing set up like for the cam bearing. Then drill it for a grease fitting if it isn't already. Hope this helps.
