I use them for anything but a short ride around town. I use the disposable foam inserts that you compress with your fingers. Work great. You can get a box of 200 for around $20.
I wear them. Not the foam ones, but not custom made either. Something in between with different inserts depending on how mcuh noise you want to block. I can still hear people talking and my music.
I have used them. I used the foam disposable ones. More often than not, if it isn't a short trip around town, I wear Shure e2c noise cancelling headphones and my ipod.
I have custom plugs, but they've gotten a bit noisey so I've switched to the compressable foam plugs.

I had some custom ones made at a bike show last year for 80 bucks and they work great. They cut the noise and wind levels down but still allow you to hear and speak normally.
Tinnitus here too, but it's been that way most of my life so it's just white noise to me except that when it is occasionally really screaming it masks out some tones I want to hear. Current pipes aren't a problem.
