Will it even look okay on the older Slim? In my eyes, it should since both designs are so close

I believe the newer softails all went up in fork diameter (41mm to 49mm?) so the triples wouldn't work. Being an all new frame design I would expect the neck of the frame is completely different.
Another option would be to use some Low Brow customs fork covers. I used a set on my old Blackline, had to cut them down a bit as they are designed for the 39mm sportster forks but they fit (just) on the 41mm forks. Gives kind of the same look as the 18 Slim.
I like what you did with yours. Not that far off from what I like on the new Slim.

Get yours powder coated black as that is the biggest difference in appearance. While it is apart, change the internals and you will end up with something better.
