I have found that most people can not hear any horn from within their cars.
It sounds loud to us but we are sitting on the bike.
Modern cars seem to be designed for a very quiet cabin and the use of the radio or cell phone while driving seems to further mask just about all external noise for driver.

That sounds dangerous to me. When you REALLY need the horn RIGHT NOW do you really want to have to wait for the horn to blast?
The whole concept has bothered me for that reason since I first read it.
I've got an air horn on mine and spent a fair amount of effort keeping the air line as short as possible to prevent lag.
Yea, I know what you mean. I've considered a bicycle bell at times, or a second horn with a second button. Not for cars, but mostly for pedestrians.
