Default Clucth Problems

Hi, new to this forum and hoping some one can give some advice. Working on my friends 1995 Road King and having clutch issues. After adjusting the clutch cold its hard to get into first (excessive grinding), As the bike gets hot the grinding gets somewhat better but not right (still grinding). I have tried backing off the adjusting push rod @ 1/2" 3/4" and 1" full turn and it still grinds bad when cold. At 1" it seems to lessen the grind when hot, but still not right. The clutch pack was replaced about a year ago and the problem still existed. The clutches removed were worn excessively some friction material worn off completely with blue heat related steels and clutches. This bike was bought new and the owner says it has been doing this for most of its life. It now has 95k on it. It seems that the clutch is not releasing completely to put into gear, shifts great up and down while moving but won't go into first while sitting still. If any one has any ideas on what to look at that can causing this problem your help is much appreciated. Thanks

Throw out bearing (on the clutch actuation side,) failing?

The 'little' bearing (accessed from the right side of the motorcycle.) Hard to find neutral and hard to shift are some of the symptoms.

When they did the clutch job, maybe one plate too many?
