Default IDS removed

I have removed the IDS from my bike. I had done this before because I was geeting a noticible rear wheel noise on the left side. I retourqued everything and put it back on. I removed it again for the same reason and pulley had a noticible wobble when I was checking everything. The bearing looks good and nothing appears out of whack that I can tell. I have a noticible wear point that you can see in the picture. I checked rear wheel alignment and all seems good. I placed the old pulley on and everything seems fine and true and the noise is gone. Anyone have any ideas what could cause this?

did you use red loctite? Is there any galling on the surface of the IDS where the bolts mate to the surface?

Isn't there supposed to be a spacer on the axle to prevent that? Maybe the bearing was pressed too far in or something.

I used red loctite the second install.
I have about 4000 miles on the IDS.
I usedt the correct spacers.
Can the bearing be pushed in too far?
