While the drivers backrest adds comfort for my crappy back and ranks very high for solo riding it's not the most important.
While adding Vance and Hines Dresser Duals with Rush mufflers is music to my ears it's not the most important thing either.
The most important thing I've added to my Ultra was the George Anderson Tour Pak relocating kit. It allowed my to have that very important drivers backrest while still giving my wife enough room to enjoy the ride with me. Ole George gave me company on the road and there's nothing likea hug coming from behind to make my day.
I think the rider's backrest did it for me. I did the Kuryakin tour pak bracket thing for my wife, but she doesn't ride that much with me. She also has the Kuryakin adjustable foot pegs, which she probably says does the most for her.
The least useful accessory has been the tour pak luggage rack. I seldom have to use it because everything I take with me fits easily into the tour pak and saddlebags. I like the looks though. Being an Ultra, I haven't found too many things to add. I have a windshield bag, LTR,RTL cam cover, windshield molding. I did do the Stage I air cleaner thing, and that was signifcant.[8D]
I get the most out of my Cruise Control. I travel 50 miles each way to work so it is a real saver on the hands. All the other stuff is just cosmetic but just as important in making this bike mine.

I also got the essentials kit for my Ultra on Saturday. And it contains the adjustable riders backrest. Hopefully i can use it while she's aboard, in conjuction with the relocator.
