Default Wheel truing problem

I am lacing and truing a new replica 16-3.00 rim to an OEM star hub. Can't get the offset (11/64 on the brake side) when lateral and concentric run out are true (0.010). I can get the offset correct but then both runouts are off. Noticed that there are a few loose nipples (25-30 inch pounds) on the brake side. All other nipples are in the 45-50+ inch pound range.

I found it best to lay the wheel on it's side, support the rim with the correct offset to the hub and then tighten the nipples in sequence. After all that, work on runouts on a stand.
