I'm running stock bars on my 07 SB and am happy with them but I recently sat on a bike with 14 inch apes and did like the feel of higher hands.
I really dont want to go with new apes but would like to do risers. My understanding is 2 inch risers are standard therefore to get a 2 inch increase I need to get a 4 inch riser.
My question is has anyone done this riser extension and if so
a. does it makes the bike handle differently
b. does that long of a riser (4 inches) looks ok on standard bars.
c. any recommendations where to get one

It CAN handle differently. Your position is different, and your posture is different, so handling should be different. It may be negligible. You can find threads on bars and risers for pics. There are so many options as to where to buy. Figure out what you want, then start searching. I'm sure many will chime in with suggestions. Good luck!
you really are better off buying buying new risers, instead of riser extensions.
you can get them all over the place, almost any lenght with pullback or straight risers. You can get them in black or chrome.
i have a few sets of cheap ebay risers and a few sets of HD (more expensive risers) and they all make the bike feel different and handle different
right now i have 6 1/2 " with 1 " pull back in a fat black riser with flatrack stlye bars
if you are running the stock mini apes, i would try getting some 4" straight risers. they look good.
Another thing you should be considering is will the stock cables and brake lines work if you go from stock (which I believe is actually 1 inch, not 2 inches) to 4 inches.
