That the first signs of a clutch slipping. In higher gears there is more load on the clutch making it easier to slip. and finally it grabs.
if readjusting your clutch doesn't solve the problem, its time for a new one. Your mileage especially with stock power does seem a bit low to need a clutch, might just be your driving style though.
