clutch lever slack

clutch lever slack
changed the primary oil and adjusted the clutch about 2000 miles ago and have had no problems. yesterday, i noticed the clutch lever was way too slack, way more free play than what i adjusted for originally. So, i took up the slack by way of the adjuster on the cable, took it out for about 15 miles in the city (a lot of clutch use), and once again there is more free play at the clutch lever.
Could the pushrod adjuster bolt have come loose or do you think something else maybe going on here...any thoughts are welcome

Wouldn't hurt to readjust clutch pack and handle. Do this on a cold engine. I use a dime to gauge the freeplay in the handle, it's worked for me over all these years on both bikes.
