Broken tooth on the gear or splines in the final drive sprocket are bad would be my pick from what you have said....I would pull the primary off and check it over.
Absolutely !!make sure the compensation nut is still tight.

Long day working today , thanks for all the ideas . I'll try to eliminate some of the easy ones by pulling the primary this weekend

Don't forget to pull the inner primary, many sins can lurk behind there, things that would make a jerking tranny blush!
