very hard shifting!...help!

very hard shifting!...help!
okay, i've gone over this thing with everything i got...
bike is VERY hard to shift, sometimes i can't even get out of 1st.... also hear some whining
anyway....i park the bike, get it in first, and can't even move the bike into neutral...but when i shut it down, it'll shift fine
i've adjusted the clutch at the primary, adjusted at the cable, and even replaced the bottom ramp in the tranny side because there was a wear spot in one of the grooves
i've done everything.... any advice or direction would be greatly appreciated!

it's a 91 .... got the side-cover off, and everything looks normal.... factory must have put an early 5 speed in here because the right side pushrod doesn't have the umbrella on it
and the throw-out bearing? ....you mean the side cover bearing behind the right side pushrod?

okay.... figured out what the throw-out bearing is....it's located on right-side pushrod in between two fat washers
seems fine...i mean, it's all together (no damage) and two washers on either side seem to move freely... are their wear limits on the bearing?
