It says right there in the shop manual to check all the fasteners for tightness at every oil change. Bet you didn't check that one, did you? So, YOUR FAULT.
Use different len's than you, but learned, after about 6 months, after I bought the bike, to keep extra lens at home. Mine have 3 tangs , molded into the back of the lens, that snap into the housing, & for some reason one of the tangs will break off & won't be long, if I dont catch it, will pop out. I check them now, every so often. Vibration is all I can figure out , what causes the tang to break off.
