Car rear-ended me on the freeway at 1:00 AM - I was doing 65 or so cruising along and got hit by a car coming up behind me doing 85-90 according to the witness. Some road rash on my lower back cuz of my leather jacket riding up due to the way I was sliding feet first, otherwise I was OK thanks to a good FF helmet - I fell free of the bike and landed on the back of my head/upper back, then slid from 65 mph down to zero on my back.
Second time a car made a left turn in front of me at an intersection. Got the helicopter ride on that one. Put me in the trauma unit for 2 days with a concussion and a little rash on my hands cuz no gloves that time. Helmet again saved me.
You can never be too careful out there is all I can say.
A dodge ram pulling a large trailer full of scrap iron pulled out right in front of me from a stop sign. I was tooling down a country highway in a 55 zone. It was an angle intersection and the driiver apparently didn't turn his head far enough. Bike and leg were fixed.
The last time I went down on asphalt was approximately forty years ago. A damned dog decide he was going to chase my motorcycle and ran in front of my front tire. He stopped in front of the bike and I didn't stop the bike. I hit the dog---handlebars flop---I hit black top and slide maybe ten yards. Dog runs home yelping. I ride home seeping blood from road rash.
In 29 years, I've experienced it all. Showing off on my sport bike, the front wheel went over the top. Loose sand, gravel or ice. A Winger changed lanes as I was passing him, bounced me into the meridian. A few racing accidents. 5 accidents involving cages. Rear-ended twice. T-boned once. Over the hood of a left turner and over the hood of a cage that pulled out in front of me. I've learned a lot from all of them. Mainly how to survive.
You don't have "Hit a Bear" in your poll. I guess it would be single vehicle. Hit a bear on Hwy 49 in the CA foothills last October with my wife. Broke a couple of ribs and did $8500 worth of damage to my 06 Ultra.

1989 woman ran a read light and totalled my 84 Kaw LTD 550 and broke my foot and road rash
2006 woman pulled out of a parking lot in front of me and totalled my 05 Road King. I got 10 staples in the back of my head and road rash
Idiot woman pulled out of a parking lot and hit me, knocked me into a car and I flew 40 feet over the roof of the car. Lost some hearing, herniated disk, back still hurts. paid off some bills though.
An off duty ambulance made a uturn in front of me right in front of a no uturn sign. In a coma for 28 days with severe brain bruise and broken bones too numerous to list. In hospital for 2 1/2 months and had to learn to walk again. Out of work for a full year while recovering.. All is well now and only have a little athritis in ankle that had been turned around 180 degrees and had to be rebuilt. Wonderful wife bought me a new bike while I was recovering to replace the totalled one.

32 yrs ago.Early spring making a downhill left hander hugging the center line.Didn't see until too late the buildup of salt and sand residue and washed out the front tire.A little slide on my left shoulder and helmet while I watched my bike slide away from me losing the clutch lever and shifter on the way.Hopefully the first and last spill/accident on the street.

Lots of spills on dirt bikes but thats just the game.
#1: Totaled 1990 Kawasaki Voyager 1200. Came out of EZ-Tag toll booth, car in left lane decided the next immediate exit was where she needed to be so she cut across all lanes to get there. Well, actually I only allowed her to get halfway across my lane before I slammed into her passenger front tire at 50mph. I welt flying over her hook and down the tollway a bit. Bike pieces were scattered all over and tollway had to be shut down. I walked away.
#2: Totaled 2005 Electraglide Classic. Light turned red and traffic stopped...except for the guy behind me. He slammed into me and pushed me into pickup truck in front of me. Then that guy got hit, who in turn hit me again and once again made me eat pickup truck tailgate. I walked away.
#3: Crossing my fingers and praying that I will never have to fill out this one.
