Default Lifter Questions

I have read through several lifter threads on this and other forums, and how this is better than that, etc., and it leads me to a few questions I hope can clear some things up for others as well as myself.....

1. What year were the "C" lifters introduced to the TC engine?
2. Was/is there a "B" lifter that preceded the "C" lifter, that was made outside the USA, and if so, What years were these installed in?
3. Where in any HD manual does it say lifters are to be replaced at certain intervals, and if so, what are those intervals?
4. Are there any other vehicles with lifter replacement intervals?
5. Does the MOCO know of or acknowledge any issue with the "C" lifters?
6. Are the failing "C" lifters I read so much about being used on stock cams, High performance cams, or both?
7. Just how many "C" lifter sets are failing in the big scheme of things? Could it be a CAM issue instead?

I am hoping some good straight answers can be obtained, as I seem to read a lot of the "My brothers cousins friend had a" type of response in a lot of threads.
