Makes sense if he has adjustable triple trees and is thinking about a sidecar.
As well , as with most transmissions the final ratio is 1:1 so what's the real sacrifice? The Pan should have adequate torque to get through the gears..
Has he the foot clutch and tank shift?

A big gap between gears means revving the snot out of it then the next gear it wants to bog because the spread is too far. A TWINKIE would have no problem with its enormous, seemingly infinite, locomotive-like power delivery but with the pan, well, we have to accept its shortcomings.....
Adding a reverse givers you less of a top end (not that a stock Pan has all that much top end). It was a bit of a learning curve for me when I went from a 4-speed to t he 3+R (in regards to installation)
Spent a couple years chasing down parts that I needed. Hardest parts weren't related to the gearbox but rather the parts needed for the top (it's for my 65 Pan). I'm running 3.5 gal tanks (tank-shift) and a hack
Knew a guy in KC who ran a reverse in a 45 flathead, was an interesting party trick but not much else.
With a 45 you are not giving up anything, just an extra gear that happens to go backwards. On a Pan you are loosing a gear, no reason if not a sidecar rig, really just a waste of time.

Had anyone ever put a ratchet top on a 3 speed with reverse?
