Most FL rear fenders are off center including my 06 Fatboy, I didn't notice that until one day my friend pointed at it. I do have a 160 rear tire and it doesn't rub the fender, one side of the tire is closer to the fender but never had problem, I don't think the rear wheel is off centerline, probably the fender itself. By the way, I didn't lower the rear, stock height, don't know if it makes a difference.
Kuma, I just measure the clearance on my rear fender, the gap on the left is a little over 1/2" and the right is 3/4". I also think the problem could be the fender struts, if the rear wheel is off center that much, it should wear the sides of the drive belt, don't you think?
Yeah, I think youre right, the belt would wear uneven.
Thanks for measuring! I was able to gently form - ok, I yanked the crap out of the fender and struts - and have 5/8" and 3/4" measuring from the "outside" of the fender to the tire where the tire would go into the wheel well. Close enough to center, looks better and should not rub again... Although this is something that will be checked again.
Dude the truck come-along thing is more than a bit crazy. WTH! Thats like trying to fix it with a sledge hammer. Im just saying.
Its got to be a spacer. I see your update says its not that but it really cant be anything else unless you've bent the fame or frame struts that the fender struts mount to with the professional come along work.

I think you'll find a lot of other fenders on fat boys look crooked. I've had 2 fat boys that were new and each fender was a little tweeked one way or another...
