It has to do with the contour of the tank. I put a set of flush gauges on my 05 Fatboy. The gauge side fit great but the fuel cap side did not. Turns out the wrong trim ring was in the package. Once I got the correct trim ring it fit perfect.

On my Deuce (HD says Fits everything except Deuce) it had a set for a Rocker...fit fine, Over time the gauge lost a small corner of the silvering and I found new gauge on flebay for $25.00 not even sure of what model it was for but it fits and works perfectly..so really not sure if there is any real difference.

I have a set of flush caps on my 06 Deluxe and the only reason the gas fill side does not fit flush is, I think I did not follow the instructions and rotate the trim ring until the ring was flush with the sticker wax still on then make note of the location and apply it (the trim ring) with the sticker exposed and ready to apply. I ordered a new trim rim ring for about $14.00 out the door and I feel it will fix the problem. Does the gas fill cap and the gauge sit at the same height, "No". There is a small difference. I love my flush fit caps and recommend them for a clean custom look. See my post from a few days ago where I installed them and posted pictures. As for part numbers get the newest revision for your bike and this fixes any problems that is apparent to Harley at this time (Like I did).
