I did the same thing. I tryed using the heel shifter for about a month and after all these years I could not get used to it. My chaps got caught on the rear heel shifterseveral timesso Idecided to remove it.

I also removed mine an capped it, looks a lot cleaner now.

It just felt unnatural upshifting with my heel pushing down.

Plus it always got in the way.

Mine fell of several times on a trip to Niagra Falls. Finally I just left it off and put a cap over the end.
If you mean the shifter, it should never fall off as long as the bolt is in place. The bolt itself keeps the shifter on the shaft, and if it falls off the shifter's threads may be cut wrong. I've seen this before both on shifters and on the reverse of the shaft with the lever that attaches the shifter rod. Loosen your shifter bolt but don't remove it, and if it can be pulled off, replace it.
