Pretty simple. I ran to Lowe’s and picjased two round plastic electric junction box covers. I used a home saw to cut two rounds and glued them together to give me the thickness I wanted. It got a little file job so it fit just right.
I cut a piece of small flat stock and drilled a couple holes to accept the bolts from the ram mount. The flat stock spans the guage hole, so it works as a camp to hold everything together.

Nice job! Does the Ram hold your phone sturdy enough without the added web strap they offer that holds it down by strapping it in from behind? I don't see it in your pics is why I ask.
