While ridingback from Sturgis I ran into a guy named Slag. He had just won second place in a contest for the best product for a Harley. I wear a size 12 and am going to pick one of these up. http://www.slickshifters.com/ Might be what your looking for. I like my heel shifter too and I also ride a Buell where I don't have one. I traded bikes with a friend and he had taken his heel shifter off and it was so wierd. Of course he really hated he couldn't get his toe under the front lever!! That Slick Shifter is sweet. A little pricey... but thats whyI spend so much time on this forum.
If you guys didn't help me spend my money... I would waste it on cheap women and expensive booze.
Now I add to my bike, Watch TV and read these posts. Life is much simpler with $200.00 shifters.

My size 13s were a bit cramped with the stock heel/toe. Got this set up from Wicked Image a while back and now have plenty of room. Don't remember how much it cost but came with a matching brave lever/peg which would not fit with my exhaust. I know it does fit with stockexhaust and just about every exhaust any of my bros had...just not mine. Made a great door knocker though!
