Happens to me from time to time. Guess I'm gonna pay more attention to how I am stopping and what I am doing when it happens. Never happened at all on my Road Glide so I am sure it has to do with the added weight of the fairing on the bars.
Man, I had the exact same problem when I first got my Standard. Here's what I did to correct the problem; As others have said, come to a final stop using your rear brake. Keep your head up with eyes looking forward; I scoot forward in the saddle a bit also, especially riding 2 up, and don't creep up to a stop. Make up your mind where you want to stop the bike, ride to that point, stick your left foot out and plant it when you stop.

Great thread. After 40 years of riding I thought I could do it all. Well, stopping this thing " smoothly" has proved to be a challange. I also got the Ride Like A Pro video and it has helped a bunch. I plan on taking his class in Oct. So, we shall see.
This is a very timely post as I am just going out to practice graceful stopping on my '08 Ultra. I will definitely practice using only the rear break for the last few feet. Great thread.
