Axle cam adjuster freeplay?

I have been having a problem keeping the axle cone nut side cam adjuster from slipping while trying to torque axle nut to spec's. Tried a couple of times can't keep the cam tight against weldment. Noticed that the adjuster had about 1/16" free play on axle, never noticed this before, but when I took rear tire off noticed that the belt tension was not as tight. Any one had a similar problem? AND if you did How did you solve it?
