From what I have seen they come lose from being overtightened...I have put hundreds of windshields on and have never had issues with the brass inserts...However I have replaced and repaired quiet a few that customers have over tightened installing windshield bags new shields etc...Have even seen a few tabs broken off of the windshield too...Harley does have some design flaws ...But this isn't one of them.

it's called edging your bets. it will costs harley X million dollars to have a quality standard of y percent. But will costs X times ten to have a quality standard of Y times two. It's cheaper for Harley to damage control the PR that result from poor quality than to actually raise it's quality standard.
Victory sales hasn't hurt Harley, but perhaps when Indian comes back into the market Harley will rethink customer satisfaction goals.
Like most companies with quality issues...Harley has a team of forum hoppers that oooh-aaah everything and keep the chats positive.

I'd have to say that the inserts for the windshield are weak. I had to buy the inserts for two late model ultras. The 07. I installed a tall HD windsield. After 200 miles I noticed the windshield was tilted toward me. I reached up and touched the windield while going 60 mph. The shield blew off and hit me in the face then skidded down the road. Inserts had pulled out.
The 2010. While changing windshields two of three inserts spun. From past experience I had already purchased three of the insert repair units.
When installing the new inserts. The slot goes in the fairing. No need to pre tapp. Use a longer 1/4 bolt with two nuts so you can lock them together and run the new insert flush with the fairing.
I have all three of mine come out. I have used epoxy and JB weld, they still came back out. worse yet when inserting the new ferrels it cracked the plastic that they screw into. said the only way to fix it is to get a new inner fairing at $500-600 plus labor. Well, not going to do that. I engineered a fix that is much stronger now and it should hold forever.
