My buddy has the RCK leather tour pack and the wife also has a bad back. She didn't find it much better then a back rest. She does however like the King Tour Pak style with the wrap around backrest, works better for her. I know it doesn't match your bike exactly but if its just for touring you could live with it. By the way my wife will not ride with a backrest once I got my detachable king tour pak for my RK. She loves it also.
I put a new seat (takeoff) from a 2011 Ultra on my 09 Road King AND THE DIFFERENCE IN RIDE WAS AMAZING! If you think the stock Road King has a smooth ride, try it with an Ultra seat. This will make her time on the bike MUCH more enjoyable. Unfortunately for me, my wife wasn't happy not having her grabrails (the Ultra seat was to wide) so we opted for a Sundowner, grabrails and a King Tourpak with wrap around backrest. When things get quiet (no intercom chatter) I find myself checking the rear views to make sure she hasn't gone to sleep or fallen off.
